A huge explosion at BP's Texas oil refinery in March 2005 killed 15 workers and injured 180. A team conducting an inquiry, led by Former US secretary of state, into the blast has accused the British-based petrochemicals giant of safety failures.
BP mistakenly interpreted improvements in personal injury rates as an indication its US refineries were safe.
The report found that prior to the Texas City tragedy, BP emphasised personal safety but not process safety. The panel also did not find any deliberate or conscious efforts on BP's part to short-circuit safety.
